Position: Community Support Specialist

In this pivotal role, you will be responsible for providing personalized support and assistance to individuals seeking to maintain autonomy in their daily lives.
Key Responsibilities
- Conduct thorough assessments of clients’ needs and preferences to create personalized support plans.
- Provide day-to-day assistance in various activities, including meal preparation, medication reminders, and personal care.
- Facilitate social and recreational activities to promote engagement and community involvement.
- Coordinate and liaise with external service providers to ensure comprehensive support for clients.
- Monitor and document clients’ progress, adjusting support plans as needed to meet evolving needs.

Who You Are

Qualifications
- Degree or diploma in social work, human services, or a related field.

- First Aid and CPR certification is preferred.
